No they're related but instead of them having an incestuous relationship they had a pseudo-marriage at a church, then Kirino (the sister) gave the ring to Kyosuke (the brother) and the relationship ended because they had fulfilled the whole 'couple until graduation' deal they had. They then went back to being normal siblings.

I find incest vile but I'd rather the producers have stuck with the incest considering they chose to go down that route in the first place, it felt like a really cheap cop out because they were scared of displeasing fans or something.

According to the author and I guess it says so more directly in the light novel than in the anime, they stayed in a relationship even after graduation.

Still a bullshit ending.
Still should have expected it given title.
