Sword plans is your starting point. they come from scribes. 1 sword plans can be combined with 15 white oak for a wood hilt or 15 iron ingots for a blade. Once you get a hilt or blade you can either combine it with 10 gems (the desc tells you what gems are compatible) or you can upgrade the hilt/blade, once they get to mecha or demonic, you can't gem them. Check the desc to see how to upgrade
Wood hilt > iron/wood > iron > mecha > demonic
iron blade > mecha > demonic
