You can choose to opt out of buying a car, it's illegal to opt out of life.
The fact is, I don't even agree with car insurance being mandatory. The only reason there isn't as much of a backlash is because a car is usually seen (Outside of specific conditions) as a luxury. With the cost of insurance being a given, and even to be fair, you can still own a car and drive it without insurance, just not on public property. (You may have to correct me on this, but I believe you can still drive it on your property without one, if you can't then I disagree with such a concept.) So it simply becomes a stipulation to drive on their property, something that you don't have to do.

Driving is a right not a privilege so the government can force people to buy insurance if the intend to use this completely voluntary action. This helps to protect property from damages that may not be repaired had the person not had insurance. Health insurance is also a privilege, but life is not. So mandating that someone purchase a product simply because of the fact that they are alive is unconstitutional.
