I'm no American military expert, but the call "Broken-Arrow" means fire on my position. Again, I'm not too sure.
However, during the Battle for Height 776 at Chechenya, Russian paratroopers called an artillery strike on their position after realizing that they will be overrun.
Still a selfless and courageous act indeed.

The second meaning of the term is to describe a nuclear incident that does not have the potential to aggravate nuclear war.

While I'm a US Army veteran, keep in mind I've never once heard this expression used during mission planning, or anything really. I'm just going to make a highly educated guess.
I would assume that the target area for a broken arrow strike is predetermined and incorporated into the operation order. It could be anywhere. Wherever during your mission planning you feel an emergency airstrike would do the most good.
In the event that shit totally hits the fan and you need to call a strike down directly on top of your own position, my guess is that something like that would have it's own unique code word.

I'm an infantryman from a country I will not specify.
If it all goes really sour we are required to say "uno-uno" on the radio which will call an artillery/mortar strike on our position.
By sour I mean if it's certain that we we'll die anyway.
If I'm not mistaken, the Russians have a similar thing.
