Stands are representative of their users' mental strength.
Steely Dan was so much of a coward, that his stand was just micrometres large.
His mental strength was so weak that the Stand Gods were impressed at how shitty he was and gave him a stand based on his inverse mental strength.
Fun Fact: Steely Dan has the second longest beatdown in the manga, lasting a whole three and a half pages.
They're more based on personality. Josuke is a kind, helpful person, who isn't afraid to lay the smackdown on someone if they insult him or his friends. So he gets a short-range stand with a power that lets him heal, and also the strength to kick someone's ass half way to hell and back (then turn them into a rock.)
Mental strength comes into play when it comes to your surviving manifesting a stand. Holly doesn't have much willpower- only kindness, so she almost died. Since DIO was killed, she could very well have the power to see stands and maybe make use of her own stand (since Joseph and Jotaro can still use their stands, this is very likely), but this has pretty much nothing to do with the main point. I'm just a nerd.
Holly got sick because her stand was inherited and she couldn't handle the base power level of a Joestar stand.
There are many characters who gained stands through natural means (being born with it) like Shizuka
a 6 month old baby whose stand is activated when she cries
and the aforementioned Steely Dan.
Despite being weak characters, these people haven't died of the psychic fever because their stands are based off of their own mental strength.
I'm positively certain there is no way to be born with a stand without it being inherited, hence why all the Joestars have stands while only DIO has been hit with the arrow. And there's still a lot of evidence pointing towards stands being based on one's personality, and them going through the psychic fever being based on how much willpower they have. Steely Dan is a coward, so he has a stand he can use without fighting, and even without having any confrontation whatsoever. Koichi finds out he works amazingly well under pressure, so his stand evolves from an egg, to Echoes act 1 and, once he finds out more about himself, to Act 2 then 3.
Of course, I could just not know what I'm talking about.

A lot of characters are implied to have been born with non-inherited stands.
I'm not saying that stands aren't based on a person's personality, that's confirmed canon. Mental strength is just the most significant factor in the functionality of a stand. If stands were based off of personality alone, then the stand fever wouldn't exist.
While most stands are based off of the person's personality such as when the person is either born with stand power, develops it out of mental strength alone, or survives the arrow. Fully inherited stands like Hermit Purple and Star Platinum aren't based off their users' personalities and are respectively based off of their ancestor stands: Jonathan's Stand and The World.
We're not given much to go on about Kakyoin's parents besides that they existed. At that point in time, it could be assume Araki hadn't fully worked out how stands worked and figured the arrow and stuff out along the way.
And sorry, I misinterpreted you. That definitely makes sense, but it still doesn't explain Joseph having a stand as weak as Hermit Purple or DIO (being as insecure and mentally unsound (I mean, c'mon. Drilling a hole into your head and forgetting you have vampire powers?) as he is) having a stand as strong as The World is.
That somewhat makes sense, though I always just assume The World could just use the powers of all existing Joestar stands, since Jonathan couldn't possibly have a stand of his own (him being dead and all.)
Kakyoin himself states that when he was younger, he was antisocial because no one could see Hierophant Green including his parents.
While DIO is mentally unsound, he's still a very strong person. I don't consider him insecure since he practically has the largest ego of all the characters in the series.
There are quite a few vampires in the series, but DIO has by far the greatest survival instinct of all of them, surviving for 100 years while most others were defeated within 1 or 2 days of being reborn.
Also, the canon reason that he doesn't use all his vampire powers is that he hasn't synchronised with Jonathan's body until he drinks Joseph's blood.

The theory of The World using the powers of the other Joestar's stands doesn't work when you consider that Star Platinum doesn't have the ability to pause time until the second last chapter. And in the Eyes of Heaven alternate universe in which DIO kills the stardust crusaders and takes over the world, he retains the ability to stop time after killing Jotaro. Also, Josuke develops Crazy Diamond during Stardust Crusaders, so if The World can stop time without Star Platinum being able to, then he would have Crazy Diamond's powers as well.
It's possible that he did have Crazy Diamond's powers, because he fixed Pucci's foot. Likely, he didn't use this power for actual combat because he just didn't know how, and couldn't use it on himself. He also used something similar to Holly's stand (the rose vines) while confronting Kakyoin.
It's also likely that Jotaro didn't even know he had a power, until he saw DIO using his. Or was keeping his power a secret so nobody knew he could stop time (explaining his exceptional speed at times (though he does have an A ranking in speed))
Crazy Diamond's power is to return things to a previous state in their existence.
Pucci was born with the deformity.
Crazy Diamond wouldn't have been able to do anything about it.

Crazy Diamond's power can be used to fix things however the user wants it to, whether that range from fucking up Jotaro's hat or turning someone into a book.
Using his time stopping power and insane vampire strength, DIO would have been able to destroy Pucci's foot and then fix it into one that works. Just a theory, because Araki hasn't stated if it's true or not, but it's a feasible theory that works with everything else.

that statement pisses me off because you're undermining the point of stands as a plot element entirely.
Araki gives the user a set of abilities and has them make the most or least of it, limited only by its description and their intelligence and mental strength.
Crazy Diamond's powers can not make things go forward in time. It is never stated or implied anywhere other than the fact that it can physically move things with its hands.
When he fucks up Jotaro's hat, he destroyed it and only fixed it half way. It does not diverge from the canon.
It is made clear from the Angelo chapter that he can break things down into their constituent elements and reform them within the same space to combine them, and that's exactly what he did to Enigma's user. It does not diverge from the canon.
Dio would be able to turn Pucci's fucked up foot into a more fucked up foot, but he wouldn't be able to turn it into a non-fucked up foot.
If Josuke can give a rock a face, a whole lot larger than what the face originally was, with the details it originally had, same voice, and with presumably working organs,
DIO can fix a fucked up foot.
You get Notorious BIG, Superfly, Cheap Trick, or Survivor.
If you die, they die.
If you kill your enemies they win
you can get a job as the best interrogator in the whole world.
Every stand has a utility, that's what I like about JJBA. Smart characters get seemingly weak stands but they use them in the most creative ways. Whereas characters like Okuyasu get stands which can literally delete space with the wave of a hand but they never use their full potential.
It's all about how you use it.
If I can't get the power to punch an English vampire into oblivion, why should I even bother?
