My philosophy professor told us about this, and there are more than just 25 hospitals were doing it.
What they mean is they are placing words in places that can only be seen if you were floating above the common eye level of the room. Some hospitals have signs strategically placed above the operating table, and others have even gone as far as to place the signs on the roof of the hospital.
So far there have been several cases of the signs being read.

How would you know if no one ever read the signs if the mentioned study's results haven't even been released yet? I may not have proof since my professor only knew that by talking to one of the researchers involved, so it may be false. There were however other cases of confirmed visual perception during an OBE; I can't remember the book or the study for it but it seemed a bit conclusive.
However, there is no scientifically proven cause of OBE's. The DMT theory has just about as much proof as the Carbon Dioxide theory.

There has been multiples studies of this kind done throughout the world.
All return the same results. No visual confirmations. Just what the person had seen as they entered the room. Specifically in 4 cases where there was a balloon in the room when the patient walked in. Then the OBE's were induced and the balloon removed. The balloon was then re-introduced to the room before the person woke up. The 4 people claimed that they were 100% sure that they saw the balloon while experiencing the "OBE" when it was clear that they could not have.
The brain constructs a virtual reality from what it has taken in before the OBE. If the person sees it before they are induced, it will appear in the "OBE". If not, then it doesn't.
The experience of being outside the body during say, an operation is the result of insufficient levels of anesthesia causing semi-awareness. The senses take in what they can and as it's not enough to relay an accurate image to the brain, objects and spaces are assumed or created by the mind itself. Not by your consciousness "leaving the body" which is impossible.

I think it means the signs are placed flat, perhaps a few inches below the ceiling (such as hanging from wires by all 4 corners or something) and they're facing upward so people in the room can't read them. You'd have to be in the space between the sign and the ceiling to see it, and in this case, they're proposing that an out-of-body "spirit" would be able to do that.
