Actually it starts with a choice, the person decides (s)he is is fat (even if actually not) and starts to dies. Sometimes it works out fine and the person will stop at healthy slim. But for a lot of people, it becomes an obsession and they see themself fat even when their limbs literaly consist of a bone and some skin. Also, anorexia is not about not eating anything. They eat, but only as much as they really need to survive (no more than 1500 calories/day). They also obsessively do sports usually. Anorexia is an obsession a little like OCD or Cleptomania.

Beg to differ. I look after men and women who suffer from major disorders and anxieties that cause anorexia. In discussions with them they understand and want to eat and be healthy, they organise action plans and set goals for the future... but as soon as they become distressed that's it, sometimes for months. We have to sedate them and feed them via a nasal-gastric tube because they'd just die.
