I think she leaves after main ops 41
then if you do enough side ops, 150 unlocks, and after than another main ops unlocks where she leaves
but if you want to keep her, you have to put a butterfly in your emblem, which will stop 41 from unlocking

or something like that, it's not like I ever researched it, because I'm on PC, I can just mod her back in and not use her because she's overpowered, and that's just boring

You just have to have full 'bond-level' with her. Eventually you will get side-mission 150. When you have done that (it's ridiculously hard IMO), you need to listen to all your audio-tapes AND fully upgrade your motherbase. Then the real final mission (46) will unlock...

I have tried it a little bit with the assist of Tarek making the textures. We got bored of it but I'd love to try it again and we three could split the profit. Having reference pictures when modelling makes things 10x easier. I'll get back to you on it.
