Well it shows Tomoya back on the day when he first met her and he talks about how he wishes he never met her so he could have never screwed up her life. And right before he truly decides he wishes he never met her he turns around and changes his mind and she takes him to see what there life would have been like had she not died.
Meanwhile in the other universe the robot (who is basically Tomoya) and the girl who is his daughter have been looking for a way out. and it's incredibly snowy so she gets sick and she dies but says they will see each other again and I believe the world begins to fall apart destroying the robot.
It then returns to the other world where Tomoya is sitting under a tree telling his daughter about Nagisa and then in basically the last second of the show you hear her voice calling them to not fall asleep outside or they will get sick.
So honestly I'm not sure if she was still dead and that was just a what-if situation or if they actually changed the past.
