Third Impact was said to occur when an angel made contact with Lilith. When it was revealed that humanity WAS the third angel and R.ei came in contact with it, Third Impact began. All humans would be reduced to the primordial soup of LCL and their souls would be encased in the Black Moon (The geofront). So when Shinji, being the unstable person he is, decides that humanity should be killed and that everyone should die, Third Impact is initiated and all the remaining humans returns to Lilith. After Shinji realizes that this wasn't the paradise he wanted and that living was much better, Third Impact is undone and he is brought back along with A.suka. All the other humans can come back if they have the will to return.

the ending scene shows that Shinji is still unstable and has much progress to make. And he's still "disgusting" in the eyes of the Second Child.
