binding of isaac challenge runs

Can someone please help me out here, ive been watching Northernlion lets play the binding of isaac for quite some time now, but only just download cheat engine so i too could try these challenge runs, but for some fucking reason i can ony have one item selected at a time, if anyone could please help that would be awesome, thank you.
