pinkyziolk you are close it is a string and it is on his head. However it also runs around the back of his head to his hand which controls the up and down motion. head controls left and right and some up and down. The string is called invisible string

theres a tiny string the same color as his hair taped to his head if ou watch the ciggarette follows the movment of his head not his hand the hand is to downplay how it follows his head
