-fag, when used in suffix form as the e-slang, isn't supposed to be an insult. It's just something used to note people as to a quality they have. A guitarist would be called a guitarfag.

America refers to North America, Central America and South America. The country of the United States of America is not just "America". It is the United States. If you are from the USA you are American in the sense that you are from the North American continent. So, when you think about it, "Americans" should be called United States of Americans.
