Well, I like genetics, so here we go. It's not the chromosomes fault on causing it, it's merely the sometimes half-assed job at replicating DNA, and there are little mistakes that occur. Those mistakes are mutations. Mutations are mostly harmless, neutral, some are advantageous, which is how natural selection works in the wild (doesn't work with humans if you were wondering, not anymore atleast), and some chromosomal mutations are harmful, like downs diseases. The type of mutation this goes with is like anon said, trisonomy 21.

Down syndrome is also know as trisomy 21 which is a extra copy to the 21st pair of chromosomes giving a human with downs 47 chromosomes.
