Everybody (me included) getting ill or injured gets help from the system, thats what it's here for. It's a good thing, so paying is totally accaptable.
But IMO getting really obese is such a long term condition, that can be prevented and should. If you're not able to care for yourself, the government intervenes (and in extreme cases bans things).
tl;dr You don't wake up one morning being obese.
