I don't think it's was ever stated that his arrhythmia was that severe. It's not a condition/illness that cuts out your life expectancy. I'm pretty sure it's something that's controllable with medicine and exercise.

The only arc he actually tries to get healthy in, is in Emi's. In all the others he just thinks it is enough with a little walk in to town once a while. Despite seeming as a really smart kid he really makes some stupid choices some times :)

I think that refers to the arrhythmia being able to send him into a heart attack at extreme/intense moments. You literally have to rebuild your heart muscles to get back into rhythm.

Katawa Shoujo is definitely a game with many life lessons, each path gives you somthing unique, whether it ends badly or not. You learn something.

Despite that I can't help but feel sorry for Hisao, all the endings contain something precious for him, something that he does not get from the other girls.

For Emi it's a healthier lifestyle, from Rin it's selfrealization in some way, and Hanako shows him well and truly how one should not be prejudiced.

These are all great things, I just wanted them to all come together some way, but I guess, in the end, Katawa Shoujo also teaches you the reality of your choices, that there are several good and bad endings, and that you unfortunately cannot have it all.
