I had a similar idea to end the Paranormal activity movies. At the very end of I guess number 5, Sam and Dean bust in, kill the demon, leave, credits roll.

What if they did the exact opposite of that? You have a movie that's all perfectly normal (and advertised as such), make it a light-hearted movie about a guy who has to find success or love or something. All the way through watching it it seems like any other light-hearted kind of movie, but there's this prevailing sense that something's ever so slightly off about what you're watching, but you can't quite put your finger on it. Then, at the end, the character realises that all the while unspeakable horror has been unfolding around him: there were always monsters in the back-ground, muffled screams, discrete blood-stains, that kind of thing (and watching it a second time with that knowledge, you'd notice all these things).
