Portals can be placed on surfaces that move in the two dimensional plane, so up down left and right. however because the 3rd dimension of movement which is the depth is conserved through portals they will dissipate if moved through that dimension, Thank you again for your purchase of the hand held portal device and please enjoy your testing

There's also a level in Portal 2 where Wheatley moves your test chamber closer to him near the end of the game. While the room is moving, you could place portals.

I'm pretty sure it depends on the axis the portal is moving on. If the Portal was on a floor for example, it could move on a two-dimensional plane (looking down). Whatever surface it's on should be able to slide around on the ground, with the portal staying intact. This is what happened when the Portals are used to cut the tubes to the deadly toxin tank. They move laterally (I think that's the word) up and down, but not forwards or backwards. Portals can move around like that, but not forwards are backwards (three-dimensionally).
And in the same game, you place portals on moving plates to cut neurotoxin tubes to prevent GLaDOS from killing you at the beginning of the game.
Wow... Valve really didn't think out the entire, "can't place portals on moving surfaces" thing through enough.
