when you're trying to make 4 sandwiches for 4 orders, all in under 45 seconds per order, it's pretty easy to miss some addons on a screen, especially since they don't show up on the screen until the next button is pushed on the register. By the time it ends up on the screen, you're halfway done and you don't look at it again.

Not necessarily true. I worked at Sonic for a good while and I only had one burger sent back to me, but only because "the cheese wasn't melted enough." And I had a pretty fast averaged time. It bugs me to no end when my order is messed up because I order all my crap plain. I'm with Pyro, how do you screw that up?
