He holds it further down the haft when he's going for a big swing, like when he uppercuts Hulk. He chokes up when just carrying it around or using magic, probably because its an oversized hammer and thus heavy as hell.

Also, a hammer made for combat wouldn't have such a sledgehammer-like head in the first place. Warhammers looked more like crowbars that something you'd pound in railroad spikes with.
