A Clockwork Orange. Very dark movie but it's one of my favorites.

I'm just curious about the context of the actual scene. What's happening? What makes me really wonder is why he's smiling, and why the person who captured him is taking such good care of his eyes.

It's also a fantastic book. Once you've watched the movie, if you liked it, I suggest that you read the book. It's even more disturbing than the movie in places.

Well, it would be a spoiler so I'm warning you now if you actually want to watch the movie.

Anyway, in this scene, Alex is being brainwashed. He was the leader of the Droogies and ended up killing a woman with a giant penis sculpture. He was sent to a correctional facility as an experiment to see if they could make him a better person.
If you watched the rest of this scene, you would notice that he's not smiling but he's grimacing and eventually begins screaming in horror and wants it to stop.

also Malcolm McDowell actually got his eyeball scratched doing this scene. Nothing like a method actor

It was seriously more of Stanley Kubrick torturing him than anything else. You know Alex's pet snake? Not in the book. Kubrick added it into the film because he knew that McDowell was scared shitless of snakes. The part where he was on the stage and that dude with the mustache was beating him up? He was actually hitting and stomping on him and ended up breaking some of McDowell's ribs... The more you know :/
