I'm pretty sure the special features say it was all Orlando Bloom's idea and Peter Jackson just went with it. Also, the scene in Return of the King when Theoden rides up and down the lines of the Rohirrim hitting their spears and shouting "DEATH!" was all Bernard Hills (Theoden) idea. Apparently all the extras and crew got really pumped up everytime he did it too.
