Rick is having a mental breakdown over the fact that his group is slowly dying off, his wife is now dead and they have an infant to take care of, on top of the general stress of the zombie apocalypse.

He is going to answer the phone, imagine it's Lori and it will continue this way for the next while.

At least that is what happens (roughly) in the comics.

Yeah in the comic books it's Lori...or rather, him imagining it's Lori. It get's really bad after a while because he had this cell phone and he'd just whip it out and start chatting with his dead wife.

At first I thought it was Carol but now I"m not so sure...also, she's not dead.
