Most commonly, the mission scripting is so ****** that merely getting into the final boss's area triggers him to appear, and the game to end when you beat him. even though none of the story up to that point which allows his existence has been done.
A good example is in Elder Scrolls IV, oblivion. You can build a ladder out of paintbrushes (they're glitchy as **** ) to enter the temple of the one by climbing over a wall, and going in that way drops you into mehrunes dagon's boss arena.
