I guess it is the fact that they are insulting the appearance, and generally, implying the stereotypes associated with them in turn apply to them. Personally, I would be more insulted that someone thought mocking my appearance would be effective.

True, but at the same time, it could be seen as something you are proud of being mocked. Like if you got a hair cut you liked but someone said it sucked, or if you lost a lot of weight, and someone called you a twig.
