That he's not able to present as female constantly destroys him as a person, even up until the final scenes where he makes a false confession to himself that he will never be able to stop crossdressing because he is sick, but that he 'accepts' his own sickness.
In reality though 'he' should be 'she', because had she grown up in a non-hostile environment, she would have received T blockers and estrogen and had her name changed to Onna-Horu.
