I don't know about this, other than that it is scarification, but I can make a guess.
The artwork is very eastern, yet the tattoo seems to be of a man rather than god. That makes me believe it is a Buddhist tattoo in origin. Some Buddhists (there are many different schools and schools within schools of Buddhism) practice denial of self to extreme levels. Inflicting pain on one's self and yet not identifying with it (it is not my True Self that is hurting but merely my physical body, not the transcendent) is one way to do that. So my guess is this individual is showing his devotion to his religion while also practicing self denial to near nirvana. Not drunk, btw.

In all fairness, I bet it looked fucking awesome after it healed.
