She thought nothing about the men, but I assume that they killed the woman that was in the middle of them and it looks as if she is staring at the woman opposite her because the two "murderers" were holding her up by the arms. The doctor knew she was dead and warned the other woman by telling her to get off the train. He saved her from possibly being murdered herself.
