Worst torture I've been able to think of:
You're buried alive in a special casket, that is completely hooked up to a ventilation system above ground, so you can breathe. At the same time, an IV is injected into you, so you are constantly provided with the nutrients you need to survive. You're strapped down so you cannot move in the slightest bit, and the coffin is completely soundproof.
You live the rest of your life in this coffin, never hearinf or seeing anything ever, only your thoughts to comfort you.

This sounds very similar to one of Dalton Trumbo's novels, Johnny Got His Gun.
"Joe, a young American soldier, is hit by a mortar shell on the last day of World War I. He lies in a hospital bed in a fate worse than death --- a quadruple amputee who has lost his arms, legs, eyes, ears, mouth and nose. He remains conscious and able to think, thereby reliving his life through strange dreams and memories, unable to distinguish whether he is awake or dreaming. He remains frustrated by his situation, until one day when Joe discovers a unique way to communicate with his caregivers." -IMDB
Dalton Trumbo wrote the book in the 30s, and he directed a movie based on it in 1971, you can find it on the pirate bay. The movie isn't really scary, but it's really sad, major feels are felt.
