We're talking about an era where doctors thought women wanting to read and write was a mental illness. These are not clever men.

Again, this is 1847 we're talking about, and the removal of a limb from a person who is not entirely numbed or unconscious via modern anesthetics being sliced into with a huge, serrated, steel blade. The assistant is literally holding down a thrashing arm/leg as it is being cut through. Also, a SPECTATOR died of shock. It could have been the patient's wife or sibling, or a med student who hadn't quite tested his mettle yet. And, again, 1847, "shock" could have been almost anything. The spectator could have had a wholly unrelated aneurysm.
