Also, Hisao tells Hanako she's the first person to see his chest scar....But he had a naked conversation with Kenji in the shower already? Is kenji's eyesight that bad or was it just a plot hole?

Depending on your choices, you experience a slightly different universe every time.
My guess is that different groups of people worked simultaneously to create the stories and couldn't perfectly synchronize them once they were compiled into the finished game before it was released. But that's ok, the game is still amazing.
