For those of you who don't get the joke, that's Bill Clinton, the president who had the sex scandal. The Nikon registers peoples heads, and the joke is that it can see his head (the tip of his dick) through the desk.

Another interpretation may be that someone is under the desk literally GIVING him head, and the Nikon can register it.
