I go into the library and correct any books I see on the shelves that are in the wrong order, upside down, or backwards. It's not my job to do this It just bugs me to no end. At home I also put all the citruses in one bowl and the apples in the other, instead of two mixed fruit bowls.

Is this OCD, because it feels like OCD.

That is just your mind wanting natural order in life; here is an example of true OCD.   
 Every day when you put on your shoes, you spend hours trying to get the laces the exact same length. Hours, just tying and untying your shoes. And when you flick the lights off, you flick them back on just in case the fuse went out. You go back and forth, off and on, till you finally leave. And then, you spend more time locking and unlocking your door, just in case the tumblers are broken.   
 That is OCD. You just want to see things in order.

Obsessive-Compulsive Disorder is mainly characterised by having Obsessions, which are repetitive thoughts about the object of obsession and Compulsions, which are repetitive actions that the individual is compelled to do. What makes this a disorder is the Obsessions and Compulsions are based around largely unimportant things, i.e. having to organise everything on your desk to true north or turning a door handle n-times before opening it.

The medical student mentioned in the question could possibly have OCD as the information given is not really detailed enough for a proper diagnosis. However, based on this information alone, I would say the student doesn't have OCD and is simply a hard-working medical student who would prefer not to be working in McDonalds in five years. The student doesn't exhibit any obsessions or compulsions and shouldn't be considered paranoid: his colleagues may take notes in ways that he would find hard to understand or study from and having a fellow medical student as a girlfriend means he's a lucky bastard with at least one (assumed) reasonably attractive girl in his classes.
