No. Just America and China. But America consists of the USA, Canada, Alaska, and Hawaii. China is a bit more expansive than it should be aswell. The Rest of the world was uninvolved in the war (Except for maybe the UK which allied itself with the US) Europe, for an example, had its governments collapsing, and radical political changes and revolutions in almost all of its countries, leaving it in endless anarchy between gangs, factions, and nations all alike.
This mostly has to do with the loss of resources and absence of oil.

Nawh. That's just what's happening in Europe, it's referenced in the earlier games and the wiki I believe. I actually don't remember where I read this though. It is irrelevant to the game, it's just part of the universe. This is why none of the Fallout Games take place anywhere outside of the USA. Except for the one battlefield-like Fallout game which was only a concept by Bethesda which took place in Europe. It's like a Fallout Battlefield game. I don't know how else to explain it.

That's a hint at more southwestern stuff.
I've just heard speculation that a DLC might take place in China.
