Words sometimes change meanings slightly when other languages borrow them. Most people I've seen saying it just mean "person who likes anime". Also, I've heard that even in Japan whether or not it's an insult depends on who's saying it.

"Otaku" doesn't mean "person that likes anime." It is an entire subculture that goes far beyond that, particularly idolization/infatuation with "2D" women as opposed to "3D." Most people that enjoy anime have nothing to do with otaku culture. Therefore the gif isn't implying anything about people that watch anime.
