In Ciel in Wonderland part 2, Ciel turns into an adult but doesn't have his eye patch, this is because Ciel doesn't have a future self being a demon, demons live forever so Ciel will never turn into an adult. His future self is non-existent which is why it can't be contracted to a demon therefor he doesn't have his mark on his eye so he doesn't need his eye patch.

Baron Kelvin/Gelwin replicated the sacrificial grounds Ciel was sacrificed/held at however the Baron was never there originally, even if he saw a picture in a newspaper he wouldn't be able to see everything, therefor it was impossible for him to know what it looked like.

Undertaker's character design and personality resembles the legend of Ankou, the personification of Death in the Briton mythology who is described as a man with long, white hair who wears a large hat to cover his face and a black robe, often wielding an old scythe. The Ankou is also said to be the "graveyard watcher".
