So I was looking to start watching through Fullmetal Alchemist, but I remember seeing people say to watch the first 9 or so episodes of the 2003 version and then going with Brotherhood. So what's the real difference between the two versions?

The first couple of episodes of Brotherhood were made under the assumption that people had already seen the 2003 version, therefore the first couple of episodes are a bit rushed.
And when people say Brotherhood follows the manga 100% that's kind of not true. There are two filler episodes, 1 and 27, although the latter is pretty good.

2003 version has an anime original story from something like episode 15 onwards.

Brotherhood follows the manga, but it has a weaker beginning, that's why I recommend watching the first 9 of the 2003 version. that being said, you can do perfectly fine just by watching brotherhood alone.

Brotherhood follows the manga entirely.

The original FMA followed the manga a little for the first ten or so episodes (I say a little because they still do some things different) then had a bunch of filler that never happened and then continued with a completely original story (which I personally thought was quite bad).

If you're planning on watch both, watch FMA before FMA:B, as it's probably the only way to enjoy it without the bias of already enjoying Brotherhood.

If you're planning on only watching one, watch Brotherhood. You don't need to watch the first 10 episodes of FMA. Brotherhood just went a little bit fast (I didn't find a problem with it) with the introduction arc compared to how FMA went.

Brotherhood follows the manga from start to finish. "Original" only follows it for a few episodes. Honestly, just watch Brotherhood. I don't think it'll confuse you that much.
