It's a picture of two different people holding hands.

They got puzzle piece tattoo's symbolizing that they fit together/were made for each other/ etc etc.

The comment is stating that they are corner pieces of a puzzle, which NEVER fit together in a puzzle.

if you flip one of them over (so the side with the heart is facing the side with the heart) they would fit together.
