In Arkham Asylum you could actually do this. In the first level where you play as Catwoman you can choose to just take the money and run, instead of saving Batman. It then rolls credits, and you have to reload a save to continue.

There was something like this in Saints Row IV. Less than halfway through the game, there's a part where Zinyak, the main villain, makes you make a decision.
The choices are something like, "Kill yourself right now, and all of your friends will live freely and safely", and "Choose to keep fighting, and doom all of your friends, and simply prolong your own death." If you choose the first one, the credits start rolling, and it goes all the way through them. After the credits, it says, "MISSION FAILED: Zinyak lied."

In the PS3 exclusive 3d Dot Game Heroes there is and optional event just outside of the castle in which you have the choice to save some dude. If you say no enough the game will "Game Over" and then come back on and say that you almost ended the game.

I remember playing Rayman for the 64 and after you get away from one guy in a level he offers you all of the money he has or everything you ever desired and if you say yes Rayman is magically poofed to a island and your belly is huge, but you still have no arms. I loved that game

In Golden Sun, you can get a game over at the beginning by choosing not to go on the quest to save the world and leaving the sanctuary. It's funny to do it, but it doesn't really punish you because you can have the option to go right back to the point where they ask you to make the decision again.

In Mario and Luigi: Dream Team, at the beginning of the summit to the maintain, you can choose to drink water from a random spring. If you choose to, the game will tell you a story and give you a fake game over screen. I honestly thought that it was real. I nearly broke it because the last time I had saved was two hours ago from that time.
