Ball moves way too fast for the guy to see it and react on time to save it. So they usually try to predict where the ball will be around.
So they kinda guess where they're gonna dive.

a good keeper can see where the players shot will go before he hits it by looking at thier run and placement of thier supporting foot when kicking the ball
Yep, that's why some players fake their movements, and make it seem like they're gonna shoot one direction, when they don't.
And it's pretty hard to do that. You might see where it's coming from, but you also gotta react on time.

depends on if the guy was outside of the box and maybe keeper went studs up?
if it was because the keeper went too recklessly but made a clean tackle it's usually at the ref's discretion if it was too dangerous and he got lucky he got away with it or if it was clean then no point in awarding opponent anything
and usually refs are taught if it's a 50/50 close call, to go in favor of not giving a penalty, so even if he went by those rules, still a bad luck ruling
