The OVA is a lot darker and meant for older audiences, while the tv series is meant for the family. They're both good, though the tv series is a lot 'feelsier' and portrays Black Jack as less of an asshole. The OVA has amazing animation.
Young Black Jack is, as the name implies, a prequel. It's not terrible, but it's not, in my humble opinion, as good as the other shows. It's a lot darker, like the OVAs and I'd recommend watching it first to avoid getting your hopes up.
And let's not forget the manga! It's also more than worth a read.
